Crossroads Care Croydon - Palliative Care Service
This is a free day and overnight service providing practical support and breaks for carers. Here's how it works:
"Our service provides respite for those who are caring for any person living in Croydon who is on their GP's Palliative Care Register (very ill and suffering from a life-limiting illness).
"We provide person-centred support for carers in the home of the person they care for. We can provide up to 8 hours a week, day or night and the service is funded by the PCT and free of charge. This service aims to help carers keep the person they care for at home for as long as possible.
"Fully trained Carers Support Workers provide practical and emotional support, personal care, administration of medication and other specialist tasks, preparation of food and assistance with eating.
"We welcome referrals from all health professionals and social workers. Carers can also make direct referrals. Contact us on 020 8688 4499."
