Services Provided By The Practice
In addition to appointments with the doctor, the practice provides the following services and clinics for patients:
Practice Nurses
The practice employs four practice nurses who are trained to provide the following
services to our patients: wound/ulcer care dressing, injections, minor infections,
blood tests and blood pressure checks, ear syringing, suture removal and general
nursing advice. In addition the nurses can give advice on HRT, family planning,
teenage health and have particular interest in asthma, diabetes, hypertension,
CHD, weight monitoring, travel vaccinations, child immunisations and smoking
cessation along with other general clinics that they run.
The nurses also carry out cervical smears and female patients will be contacted
when they are due to be screened.
District Nurses
There is a district nurse team attached to the practice. They provide nursing
care for those patients of all age groups in their own home, to those patients
who are housebound and need long term care, wound/ulcer dressing, insulin injections
and care of patients who may be terminally ill.
The district nurses liaise with your doctor, other members of the healthcare
team and social services to ensure the best possible care is provided. They can
be contacted on 020 8714 2750 but your GP will have to refer you.
Health Visitors
There are health visitors attached to the practice and they give advice on
preventative health. They have expertise in a wide range of healthcare fields,
particularly those concerning expectant mothers and young children. They can

Community Midwife - by appointment
The community midwife is no longer attached to the practice and the antenatal
clinic is now run from the Byron Centre (020 8660 3915) The midwives can be contacted
on 020 8401 3171 for advice. We run an antenatal clinic at the practice for those
expectant mothers who need to been seen by the GP and you will be told at what
weeks you need to see the GP. This is run on a Monday from 10.00am - 2.00pm.
Dietician - by appointment
The practice has a dietician attached to the practice who is here every 1st
and 3rd Tuesday of each month and will give advice on diet. If the doctor feels
you need to see the dietician you will be referred to reception to make an appointment.
What Is The Minor Illness Clinic?
This is a nurse-led clinic, which is run Monday to Thursday. No appointment
is necessary - you can walk into the practice between 8.00 - 10.00am or you can
ring between 8.30 - 9.30am and you will be given an estimated time to attend.
The receptionist may ask you for a few details to ensure this appointment will
be suitable for you. The nurse is able to prescribe certain medication and antibiotics
if she feels that it is appropriate. Please see below for the type of minor ailments
that can be dealt with by the nurse.

Monday 10.00am - 2.00pm (with Dr Harcourt)
Diabetic
Thursday 1.00 - 4.00pm (with Dr Cruickshank or the nurse)
Asthma
By appointment with the nurse.
Baby Clinic
Wednesday 1.00 - 4.00pm (with Dr Kelvin or the nurse)
The clinic
is run by the GP who will conduct 8-week checks and then the nurses will give
first immunisations. The rest of the clinic is nurse-led and the nurses give
other child immunisations as per the schedule below.

Flu Clinic
October - December
The practice offers eligible patients a free flu
vaccination. Please check to see if you are eligible and then make an appointment.
Travel Immunisation
Tuesday 3.30 - 6.30pm
If you require travel vaccinations please make your appointment six weeks prior
to travel. A charge will be made for certain immunisations that are not covered
by the NHS. (See Travel Clinic below.)
Cryotherapy Clinic
By appointment.
Every three weeks the practice holds a cryotherapy clinic where liquid nitrogen
is used to freeze off minor growths such as warts etc. If the doctor feels that
you need to attend this clinic you will be asked to make an appointment at reception.
Ischaemic Heart Disease
By appointment.
The practice holds clinics for those patients suffering from heart disease. Patients will be invited to attend or be referred by their doctor.
Hypertension Clinic
By appointment.
The practice operates a hypertension clinic for patients with high blood pressure. You will be invited to attend or be referred by your doctor.
Smoking Cessation
The practice operates a group smoking cessation clinic which is run by the nurse. Please ask reception for detail on how to book your appointment.
Weight Monitoring
If you have a weight problem, attendance at our informal group weight management
session will provide an introduction to our weight management programme and help
to support you to lose weight. After the group session you are monitored on a
one-to-one basis by either the nurse or healthcare assistant. Please ask for
details about the clinic at reception.
Non-NHS Services
Certain services such as private sick notes, insurance claim forms, passport
applications, some medical examinations and travel vaccinations are not covered
by the NHS and the practice will charge a fee for the provision of these services
in line with the recommended BMA rates. For more information on services and
fees payable, please see the notice in the waiting room or ask at reception.
Travel Clinic
We aim to provide an efficient comprehensive service to our patients for their
travel requirements. We have been running the clinic for many years and have
significant experience. Over the years the number of patients travelling to more
exotic destinations and travelling to more than one destination has dramatically
increased. This has increased the complexity of the clinic and the advice we
give. The advice and many of the immunisations are free to patients.
Booking
Bookings for travel clinic may only be made when a booking form has been completed.
This enables the nursing staff to make an assessment prior to your appointment
as advice for some areas changes regularly. The form is available to download
(please click here) or from reception. Please complete ONE form
for EACH patient and either bring in person or fax to the surgery. When we have
this form we can make your appointment.
The travel clinic does not have infinite capacity and many immunisations require
a course of injections. As soon as you have made arrangements for a trip please
contact us and make an appointment (preferably six weeks prior to travel). From
time to time patients request urgent slots to be seen. If we have an available
appointment this is possible but if not you will need to make your arrangements
via an alternative clinic. Details are below. PLEASE do not ask to be excepted
from this arrangement.
If you need to discuss contraception or on going medical problems for your
trip please make a separate appointment with the nurse or GP to discuss these
to allow the clinic to run to time.
Fees
Many of the immunisations for travel are available on the NHS. However, some
medications are not available on the NHS for travel purposes. Please find the
costs of those items, which are NOT available on the NHS and the relevant fee.
Payments are accepted by cheque or cash. (Please make cheques payable to Old
Coulsdon Medical Practice.)
